Overview

Responsible for the implementation of Physical Therapy treatment programs as established by licensed P.T.  Communicates status of patient to responsible P.T. on regular basis.  Provides assistance to P.T. as needed in performing certain tests, assessment procedures, and complex treatments.

Responsibilities

  1. Administers Physical Therapy  treatment programs as planned and directed by licensed P.T. 
  2. Implements appropriate treatment utilizing modalities, manual techniques, therapeutic exercise, gait and  functional mobility training, patient and family/ careperson education, and specialized intervention as indicated. 
  3. Completes Skilled Nursing Facility resident quarterly screens and MDS documentation as required. 
  4. Educates patient and carepersons in use and care of therapeutic equipment:  wheelchairs, assistive devices, othotic and prosthetic devices, etc.   
  5. Provides patient education in all areas of  physical therapy treatment to maximize patient independence and assist in the efficiency of  the rehabilitation process. 
  6. Provides assistance to P.T. as needed in performing certain tests, assessment procedures, and complex treatments.  
  7. Maintains open communication with responsible P.T. regarding patient's performance/ progress. 
  8. Documents daily treatment and supplementary documentation as indicated thoroughly and accurately to provide effective communication with physicians and members of the rehabilitation team.  
  9. Adapts physical therapy treatment to ensure optimal learning in patients with altered cognition or communication.
  10. Adapts physical therapy treatment as needed to ensure that patients of all age groups and cultures receive optimum benefit of treatment. 
  11. Participates in multidisciplinary treatment and discharge planning.
  12. Communicates with medical/ allied health personnel regarding patient's progress, needs, and discharge planning. 
  13. Directs and supervises Rehab Aides, P.T.A. students, and other support personnel as indicated by New York State licensure.
  14. Other duties as assigned.

Qualifications

  1. A.S. degree in Physical Therapy from an accredited institution and licensure of licensure eligibility in New York State
  2. Ability to physically treat an average of 15 inpatients/ 18 outpatients per day and complete accurate and legible documentation as per established guidelines
  3. Ablity to qualitatively treat all age groups and  diagnostic categories in both an inpatient and outpatient setting 
  4. Ability to communicate effectively with patients and caretakers regarding anticipated P.T. treatment plan, level of expected patient participation, and expected outcome 
  5. Ability to communicate effectively with the public and the medical/ CVPH staff as a member of the Rehabilitation team

 

As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
