The Clinical Education / Intake Manager is responsible for working collaboratively across the organization with leadership and staff to ensure overall orientation, education and training for staff development and patient care delivery both in the classroom and at the bedside.  This individual is also responsible for the coordination and accuracy of Resident Minimum Data Set (MDS) and resident intake coordination.  This individual displays competence in Adult Learning Principles and the 5 core health care competencies of interdisciplinary team work, quality improvement, evidence-based practice, patient-focused care and informatics.  He/she is a clinical and professional role model and demonstrates commitment to lifelong learning.  Responsibilities include: educational program planning and delivery, compliance with regulatory standards, setting and maintain standards of nursing care and practice, coordinating services, coaching for development and providing for professional growth of staff, coordination and accuracy of MDS and resident intake coordination.  The Clinical Education / Intake Manager works hours as needed to meet the needs of staff and organization to include consistent, visible presence on off shifts, weekends and holidays.



  • Facilitates daily SNF IDT meeting and manages tracking sheet for resident care concerns requiring subsequent follow up.
  • Creates an environment which recognizes and values interdisciplinary roles around a patient/family centered model.
  • Provides information in a clear, transparent manner and seeks continual feedback.
  • Builds interdisciplinary relationships focused on a collaboration, conflict, resolution, and patient centered care.
  • Develops credibility and effective relationships within the healthcare system.
  • Partners with Rehabilitation to set assessment reference dates.
  • Supports hospital’s and department’s recruitment/retention plan in collaboration with staff and interdisciplinary teams.

Communication and Relationship Building:

  • Utilizes active listening skills and is responsive to feedback and concerns.
  • Asserts views using non-threatening, non-judgmental methods.
  • Confirms positive behaviors and addresses/manages negative behaviors of staff, physicians, peers and other hospital employees.
  • Represents the organization to constituents in the community in a positive manner.
  • Assists in identifying education and orientation needs of existing and potential staff.
  • Establishes an ongoing coaching/mentoring relationship with staff.
  • Participates in instruction/training of staff
  • Supports CVPH in collaboration with affiliated professional schools.
  • Designs, implements, and evaluates education programs using various teaching/learning methodologies.

Informatics/Business Skills:

  • Maintains the facility’s CMS-802 Census form to ensure DOH survey readiness.
  • Implements recommendations from external consultants.
  • Maintains the Harmony (HC) recommended “hot list” to track resident health conditions requiring skilled nursing care.
  • Coordinates restorative program.
  • Completes a comprehensive Skilled Nursing note on all new sub-acute, skilled admissions.
  • Responsible for completion of MDS and maintains a tracking system for MDS assessments.
  • Ensures data is entered into Sigma system accurately and according to NYS and CMS requirements.
  • Participates in the evaluation and education of information systems in the practice setting.
  • Assists in the completion of staff performance evaluations in the timeframe as outlined by CVPH’s policies.

Leadership/Professional Practice Skills/Evidence Base Skills:

  • Demonstrates accountability for job performance and supports the Mission, Vision and Values of the organization.
  • Complies with the NYS Department of Health/CMS regulations and other applicable standards.
  • Ensures Care Providers practice in roles allowed by NYS Practice Act, the ANA Code of Ethics, The Joint Commission, the NYS Department of Health and any other applicable governing laws.
  • Works closely with Case Managers and Physicians to coordinate admissions to the Skilled Nursing Facility.
  • Partners with financial counselors to verify benefits and skilled nursing coverage.
  • Reviews potential admissions with the Medical Director and SNF director of Nursing and effectively communicates to the SNF team on which residents will be coming to the unit.
  • Assists in the organizational plan to ensure staff orientation, competence, and professional development. Ensures accurate recordkeeping is maintained.
  • Assists in providing clear and consistent performance feedback to staff.
  • Provides educational program planning; implementation, evaluation to individuals and teams and in a variety of formats. (lecture, bedside, simulation)

Quality and Patient-Focused Care:

  • Ensure accurate/timely completion of Patient Intake.
  • Is an active member of the Facility’s QA (QAPI) committee and provides monthly, quarterly QA reports at the monthly and Quarterly QA meetings.
  • Promotes SNF’s performance improvement initiatives.
  • Supports the organization-wide patient safety program.
  • Articulates the organizational and departmental/unit quality improvement goals and objectives.
  • Reviews and updates resident care plans and CNA care cards as needed.
  • Coordinates resident care planning meetings and follows up on the completion of care plans.
  • Assists in the development, implementation, and evaluation of a performance improvement program which incorporates clinical data, service excellence, quality improvement and risk management.
  • Demonstrates service excellence at all times and always puts resident care needs first.
  • Rounds weekly on LTC residents and daily on SAR residents.


    1. BSN or completion of BSN by Spring 2018 required.  MSN preferred.
    2. Current NYS Registerd Nurse required.
    3. Current MDS certification required.

    4. Specialty certifications e.g. Dementia certified, wound care certification preferred.

    5. Must have impeccable interpersonal skills (both verbal and written).
    6. 3-5 years of long term care experience required.
    7. Case Management experience preferred.
