The SNF Director of Nursing has the authority, responsibility, and accountability for the overall program operations and resident care provided in the clinical area. This individual demonstrates leadership and is responsible to work collaboratively in an ever-changing environment to assure the delivery of quality resident care. Responsibilities include organizational planning, compliance with regulatory standards, fiscal management, setting and maintaining standards of nursing care and practice, coordinating services, and providing for the professional growth of staff.


Quality/ Patient Centered Care

  1. Acts as member of unit Interdisciplinary team.
  2. Makes unit outcome based rounds.
  3. Demonstrates a commitment to the highest level of personal and professional ethics and legal compliance.
  4. Acts as a role model for direct care staff on SNF.
  5. Assists direct reports with continuous quality improvements utilizing state and national standards.

Evidence Based Practice

  1. Maintains the system and facilitates the process for developing, implementing and evaluating standards of care and standards of practice based on the discreet resident population, relevant professional standards, and current research.
  2. Assures that patient care is delivered utilizing an appropriate model of care delivery for the resident population and that all care providers practice in roles as allowed by the State Practice Act and for which they have been educated and trained.
  3. Conducts research for long term care program development utilizing appropriate resources
  4. Assures patient, staff and education plans are appropriate.
  5. Identifies and implements competencies as appropriate to SNF.

Leadership / Professional Practice

  1. Develops and facilitates a model of collaborative professional practice.
  2. Promotes a positive view of long term care.
  3. Holds self and others accountable.
  4. Ensures compliance with NYS codes, rules and regulations for long term care.

Communication/ Teamwork

  1. Welcomes all new members of the CVPH team (e.g., staff, physicians, volunteers, students) and
  2. Demonstrate thoroughness and patience when orienting them.
  3. Provides consistent and complete handoffs of communication regarding the patient (e.g. Situation,
  4. Background, Assessment, Recommendation
  5. Collaborates with nursing faculty in planning, implementing, and evaluating clinical experience for students.
  6. Acts as clinical resource for long term services

Business Skills

  1. Maintains working knowledge of PRIs, MDS+, Case Mix Index and impact on financial aspects of
  2. Maintains staffing pattern based on census and resident acuity.
  3. Monitors usage of specialty equipment.
  4. Works with SNF Administrator to develop, implement and monitor a sound fiscal plan for the department based on relevant data and best practice (operating/capital).
  5. Reviews monthly operating reports, and submits financial analysis/justification for variances with written action plan.
  6. Maintains an equal to or less than 5% variance in salary and non-salary expenses and HPPD (Hour Per Patient Day).
  7. Develops, implements, and monitors staffing plan which assesses patient need and assigns nursing personnel to provide care to patients based on the levels of staff expertise available.
  8. Educates patient care team members on financial implications of patient care decisions.


  1. BSN or completion of BSN by Spring 2018 required. 
  2. MSN preferred.
  3. Current RN licensure in NYS required.
  4. Long Term Care certification required.
  5. Must demonstrate leadership and managerial abilities with good oral and written communication skills.

  6. Must meet requiremments of pre-employment screening and physical.

  7. Minimum 5 years Long Term Care clinical experience and 3 years supervisory experience preferred.
  8. Microsoft Office, Policy Manager, Soarian, Bed Tracking, Transport Tracking, Healthstream, HRMS, Smart Square, Soft Med, Pyxis, MSDS, Medkeeper, PCM Auditor, Med Series 4, Nash, Comm Tech Messenger computer experience required.

As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
