  • Overview

    The Peer Education Coordinator is responsible for the development and coordination of the Peer Education Program within established program guidelines.  The Peer Education Coordinator will work with Peer Educators to create outreach and education opportunities throughout the entire service area, developing and providing public education, awareness and active outreach in Breast and Prostate Cancers, leading to an increase in screenings and early detection of cancer.

    Responsibilities

    1. Oversee the overall implementation of the Peer Education Grant..
    2. Work closely with Peer Educator (s) in the coordination and development of Outreach and Peer led initiatives.
    3. Organize and coordinate activities and work flow for the This may include but is not limited to scheduling of activities to accomplish the grant’s goals and objectives.
    4. Provide assistance and guidance to peer educator(s) on work plan goals and objectives.
    5. Provide orientation and training for peer educator(s) as needed.
    6. Prepare and submit required reports to NYSDOH as needed.
    7. Identify community service resources. Build and maintain strong relationships with networks of community resources.
    8. Share knowledge of community resources for health care, social services and additional support services, providing referrals to clients to health care and social services.
    9. Assist clients with accessing services and other resources, providing additional support and follow-up as needed.
    10. Represent the program and participate in meetings with other local agencies and programs.
    11. Promote appropriate health information within the community by identifying key community leaders andorganizations, as well as other community characteristics, to be able to find and engage target population.
    1. Educate community leaders on the importance of screening and accessing services to help promote cancer screening in their communities.
    2. Employ effective outreach methods and strategies to reach target populations, conveying health information in appropriate and accessible ways while motivating clients to engage in behavior change and access health services.
    3. Assist in the development of program budget and work plan. When necessary assist in submission of RFP’s for continued program funding.
    4. Act as the liaison between CVPH and the NYSDOH.
    5. Meet regularly with Director, Oncology Outreach providing progress reports on efforts.
    6. Monitor budget on a monthly basis to ensure expenses are on track and are in alignment with goals and objectives of the work plan.
    7. Prepare monthly billing reports and vouchers to submit to NYSDOH for payment of services and infrastructure costs, in a timely manner.
    8. Attend all required training and educational services of NYSDOH; including regional and state meetings.
    9. Other duties as assigned.

     

    Qualifications

    1. Three (3) years experience in community outreach required.
    2. Prior work in a medical setting and medical terminology preferred.
    3. One (1) year experience in marketing required; Two (2) years experience in marketing preferred.
    4. Understanding of Cancer Screening and health risks associated with it.
    5. Associate degree in Health, Human Services, Social Services or related field required, Bachelors degree in Health, Human Services, Social Services or related field preferred.
    6. Computer knowledge required.
    7. Excellent communication skills required.
    8. Excellent organizational skills required.
    9. Public speaking/presentation experience required.
    10. Valid NYS drivers license required.
    11. Ability to make appropriate decisions with minimal supervision.

    As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
