CNA - Skilled Nursing Facility

Skilled Nursing Facility Certified Nurse Aides work with our residents on SNF.

 

The Certified Nurse Aide provides direct care to residents to include resident hygiene and activities of daily living under the direction and supervision of a Registered Nurse.  The Certified Nurse Aide performs those direct care activities which are allowed to be delegated to unlicensed personnel under the NYS Practice Act, regulations, hospital policies, New York State Long Term Care regulations and standards met by Nursing Services.  The Certified Nurse Aide also performs support functions for the unit/department as assigned by the appropriate Registered Nurse and/or manager.

Increased Hourly base pay range: $19.81 - $28.72. Full-time CNA positions may be eligible for a $4,000 sign-on bonus - prorated for part-time!

1. As directed, takes and records temperature, pulse, respiration, weight, height, intake/output measurements, blood pressure, and pulse oximetry.
2. Supports patient in all activities of daily living to include maintaining patients' clothing and other personal property (including prosthetic devices, hearing aids, dentures, contact lenses, etc.).
3. Provides care to the patient requiring specimen collection.
4. Documents/Communicates care consistently and timely to team leader RN to include any changes observed in condition or behavior of patient.
5. Assists in maintaining a safe, clean, stocked and functional environment by reporting equipment that is out of order.
6. Provides care according to standards to the patient requiring restraints/ protective/supportive devices, fall protocol, wandering/elopement/ suicide/ abduction and seizure precautions.
7. Provides care to the patient with skin care and nutritional needs.
8. Performs in crisis and emergency (e.g. cardiopulmonary arrest and the Heimlich maneuver) while maintaining a positive calm approach.
9. Assists with positioning patient, lifting and turning patient with proper body mechanics, applying/utilizing special equipment as necessary, and ambulating the patient as directed.
10. Provides care using infection control practices as outlined in Medical Center policies/procedures (e.g. Universal Standard Precautions and Isolation Techniques).
11. Demonstrates knowledge/ability to effectively use applicable devices and technology (computer patient lift, urinary catheter, etc).
12. Assists with care for the newly admitted, transferred (including pre/post procedure patient), and discharged patient or expired patient (post mortem care).
13. Strives to maintain a patient’s maximum level of independence and encourages family participation.
14. Assists other team members in accomplishment of patient care and/or unit tasks.
15. Supports fluctuation in patient care needs by floating/overtime when required.
16. Applies concepts of restorative PT/OT care per therapists written guidelines.
17. Attends annual training on and demonstrates understanding of Resident abuse detection and reporting as required by NYSCRR, including handling of Residents' personal property and belongings.
18. Attends staff meetings, education programs and care plan meetings as required.
19. Other duties as assigned.

1. High school diploma or GED equivalent must be obtained within 2 years.
2. Completion of a NYS approved Long Term Care training program and hold current NYS certification required.
3. Previous experience as a nursing assistant preferred.
4. Meets requirements of pre-employment screening and physical.
