Overview

The Patient Logistics Assistant is responsible for assisting direct care givers in various duties associated with Clinical Assistants, Environmental Services, Nutrition Services and Transport. Patient Logistics Support Assistants/Aides provide timely and accurate communication to assist in maintaining a harmonious, team oriented, customer friendly environment.  Various tasks associated with non-clinical duties such as room prep, changing linens, ambulation. Distribution of meal trays and feeding, sitting with patients, cleaning patient rooms and bathrooms and transporting patients to and from tests or discharge.

Special Criteria Details

External candidates are  eligible for a one-time signing bonus of $4,000 (prorated for part time roles).  Please note that current University of Vermont Health Network employees are excluded from eligibility for this bonus. Additional terms and conditions apply. 

Responsibilities

  1. Performs patient room and unit readiness tasks.
    • Ensures that the patient room is clean and orderly, and contains all necessary equipment prior to arrival of a patient.
    • Ensures that the patient room is stripped and ready for turnover clean after a patient is discharged.
    • Cleans and removes all patient equipment upon discharge.
    • As directed by the supervisor or Charge Nurse, is responsible for general unit maintenance and readiness tasks.
  1. Provides care using infection control practices as outlined in the Medical Center policies/procedures (e.g.) Universal Standard Precautions and Isolation Techniques).
    • Daily cleaning of patient rooms and bathrooms.
    • Cleaning of transport and any shared equipment.
    • Using appropriate PPE and providing appropriate PPE when transporting the patient.
    • Providing basic hygiene to the patient.
    • Assists patients with activities of daily living.
    • Handling food trays and providing support to patients during meal time.
  1. Functions as an integral part of the patient care team for the unit assigned.
    • Conveys compassion and patient-centered focus with patients, family, care team and visitors.
    • Documents and communicates care in an accurate, consistent and timely manner to RN and or Charge Nurse to include any changes observed in condition or behavior of patient.
    • Collaborates in the provision of ancillary support services such as Respiratory Therapy, Physical and Occupational Therapy, Speech Therapy, etc.
    • Assists other team members where needed in the accomplishment of patient care or unit tasks by assisting in positioning, pivoting, lifting, and ambulating, transferring patients to and from bed and stretcher with appropriate devices.
    • Weighs patients in compliance with established policy.
  1. Provides for a Safe Work and Patient Environment.
    • Provides for orderly transfer of patient to home, other units, nursing home, other facilities or morgue utilizing approved methods and safe patient handling equipment.
    • Remains calm and professional in crisis and emergency situations to include performing CPR and the Heimlich maneuver.
    • Accurately completes verbal and written hand of care as required for task completing.
    • Safely maintains and utilizes all tools/equipment/chemicals required to accomplish all daily tasks or assignments.
    • Demonstrates to secure all patient information on electronic devices by following the computer work station security policy.
    • Demonstrates knowledge related to Patient identification and color coded safety alerts policy.
    • Demonstrates knowledge and practices according to the safe patient handling and mobility program policy.
  1. Sets clear expectations with patient about the service.
    • Sets clear expectations with patient about service through positive communication and response.
    • Helps maintain a quiet and peaceful environment that promotes patient rest and healing by following the Noise Reduction Policy.
    • Reinforces facility’s commitment to being responsive to patients’ needs by letting patient know they will do their best to make them comfortable during their stay.
    • Encourages patient to ask questions and them know if there’s more they could be doing.
    • When teaching a patient how to operate the call light system, lets patient and (if present) family members know how long it should take before their call is answered, who will answer the call and who they can expect to respond to a call.
    • When answering a call button, confirms patient request and provides and estimated response time.
    • Answers patient requests in an appropriate manner and assures timely follow up.
    • Explains immediate next steps in plan of care and when they will be back to see the patient.
    • Routinely meets or exceeds expectations of the patients.
  1. Provides hand off of care following the Hand Off of Patient Care Responsibilities Policy.
    • Meets with clinical partners formally at the start and end of shift for report and assignment information.
    • Partners with the RN and CA (clinical partners) during bedside hand off of care and hourly rounding.
    • Collaborates throughout the shift with clinical partners on progress and care of patients.
  1. Maintains unit stations in a stocked and efficient manner.
    • Ensures all patient rooms are stocked of paper and soap products.
    • Removes wastes and linens as needed to maintain an environment where odors are controlled and unobtrusive.
    • Performs unit rounds collecting shared equipment and keeps storage rooms and equipment rooms organized and replenished as needed.
    • Cleans and returns wheel chairs and stretchers to home locations.
    • Completes refrigerator checks in unit kitchenettes maintaining appropriate labeling, dates, rotation of stock, and cleanliness.
    • Maintaining supply and assists in maintaining cleanliness in unit Environmental Service Closet.
  1. Supports the overall Mission of CVPH by supporting additional functions/needs or tasks that arise through physical or technical growth.
  2. Other duties as assigned.

Qualifications

  1. High School Diploma or GED equivalent required.
  2. Ability to use basic computer skills required.
  3. Competancy to perform CPR and Heimlich Manuever.
  4. Ability to work independently, handle interruptions, take direction and prioritize.
  5. Must work well as a team member.
  6. Prior experience in a patient setting prefered.
  7. A high level of sensitivity, interpersonal skills and effective verbal communication skills required.
  8. Ability to read,write and comprehend medical terminology, labels and directions.
  9. Ability to do basic math skills for mixing chemicals.
