The Clerk IV performs receptionist and clerk duties in the department.  Functions as the central communication source for the department process. Maintains and organizes paper related to patient care and medical record supplies, distributes mail and information to staff, physicians and patients.  Transcribes both manually and into the computer patient and unit information.  Directs both written and verbal communication to appropriate people.  Zerox projects for staff and physicians.  Answers the telephone and call system.  Responsible for the timely, accurate and clear transfer of communications including physician orders.  Develops and maintains harmonious, customer friendly environment.  Monitors unit cameras and individuals entering and leaving the unit.


  1. Provides for safe, orderly and efficient communication and functioning of the unit's in all situations to include crisis and STAT needs.
  2. Provides for orderly admission, transfer and discharge of patients.
  3. Supports the provision of ancillary support services to assist patients as needed (e.g. respiratory therapy, PT/ OT, speech therapy, discharge planning, telecommunications, EKG, materials management, etc).
  4. Performs duties using proper infection control practices as outlined in Medical Center policies/procedures/protocols.
  5. Demonstrates knowledge/ability to effectively use the order entry system, message center and other applicable computer programs.
  6. Provides for precise, timely and accurate transcription of all orders per CVPH guidelines..
  7. Demonstrates knowledge and ability to use technology (e.g. computer, fax machine) per policies.
  8. Maintains patient records in appropriate order and with necessary and accurate posting, including thinning charts according to policy.
  9. Maintains assignment boards, bulletin boards and other communication sources including the recording and delivering of messages.
  10. Demonstrates positive telephone etiquette.
  11. Supports fluctuation in patient care needs by floating/ overtime when required.
  12. Provide general unit clerical support (i.e. filing, correspondence, office/unit organization, data entry, keeps station organized, utilizes MHU legal documents as needed). Faxing information to agencies/hospitals as directed.
  13. Monitors cameras and access to and from the unit.
  14. Obtain medical records.
  15. Provides patient census when requested by staff and/or physicians.
  16. Assist with transport of patients to other destinations, as directed.
  17. Demonstrates ability to effectively use verbal de-escalation techniques/PMCS techniques.
  18. Assure completion and gathering of MD progress notes to use for billing submission.
  19. Monitoring of meals, 1:1 supervision of patient activity as directed.
  20. Other duties as assigned.


  1. Must take Medical Terminology Exam and pass if no previous Unit Clerk experience.
  2. High school graduate and ability to read, write and comprehend medical terminology and willingness to use positive communication skills.
  3. Previous experience in clerical or receptionist position desired; hospital experience or satisfactory completion of training program preferred.
  4. Able to perform detailed assignments, work under pressure, and relate to patients, hospital staff, physicians, and visitors. Demonstrates ability to prioritize and complete multiple job responsibilities in a timely manner.
  5. Competent in use of language when communicating via telephone.
  6. Able to perform computer skills following appropriate training on hospital information system.
  7. Meets requirement for pre-employment screening and physical exam.
  8. Ability to work independently under stressful conditions and handle calmly and positively multiple interruptions.
  9. Mental Health experience preferred.
  10. Provides documentation within first 30 days of employment indicating no evidence of child abuse or maltreatment through New York Central Registry.
  11. Must have current CPR or obtain within six (6) months of employment.
