An Operating Room Technician – (ORT) is a member of the surgical team who acts as a primary scrub person and must be technically adept by handling the instruments, supplies, and equipment necessary before, during and after the surgical procedure. ORTs are required to perform all cases that surgeons perform. He/She may also circulate under the direct supervision of a Registered Nurse. The ORT must be able to take call as scheduled, anticipate the needs of the surgeon, have excellent adaptation and coping skills, have excellent organizational skills and be a team player. Must be able to stand for prolonged periods of time. Must be able to lift patient and equipment with assistance of other personnel or without as need dictates. ORT must have excellent communication skills and maintains asepsis at all times. Must be able to serve as a resource person for certain specialties. Will assist directly with the orientation, training and serve as a mentor for new ORTs. Must be able to work closely with RNs in room and management about instrumentation and equipment for cases. Also will assist in education of all ORTs. ORTs will work primarily at CVPH main campus with assignments to Ambulatory Surgery Center.

S(he) is responsible to provide health care services as licensed professionals working within their defined scope of practice. This position requires regular use of independent judgment.

The Radiologic Technologist works under the supervision of the Practice Supervisor to ensure the accurate and timely completion of the duties and responsibilities of the position. S(he) is responsible to obtain optimal x-ray images as required for physician diagnosis while maintaining efficient patient flows. S(he) must regularly use independent judgment to interpret and apply policies, procedures and practices.

Under the direction of the Supervisor, Food and Nutrition, the FSW performs a variety of food service functions including patient tray assembly and distribution, and dish room activities. Adheres to general sanitation practices when handling food and equipment. Responsible for the proper operation, handling and cleaning of equipment used in performing duties.
Under the direction of the Supervisor, Food and Nutrition, the PSA performs a variety of patient/resident services. These functions include but are not limited to the following, delivery of meals, nourishments or floor stock, assisting a patient/resident with menu selection, assistance with tray set-up, opening containers, , communicating any related issues to the nutrition department or nursing station as appropriate. Performs a variety of food service functions including patient tray assembly and distribution. Adheres to general sanitation practices when handling food and equipment. Responsible for the proper operation, handling, and cleaning of equipment used in performing duties.

The billing and customer service representative processes charge and/or payment information, enters into database, completes billing process, and assists with daily reconciliation. Responsible for billing and collecting inpatient and outpatient Hospital and physician billing accounts according to established policy and procedure. In addition, is responsible for accurately maintaining account balances on a daily basis, as well as staying current with constantly changing rules and regulations in order to ensure prompt collection of monies due. Must maintain files for accurate follow-up on unpaid accounts.

The Certified Pulmonary Function Technologist/Licensed Practice Nurse performs a variety of pulmonary diagnostic tests including, but not limited to, pulmonary function, oximetry, FENO testing, home sleep studies, and administration of DME. The CPFT/LPN performs routine quality control testing on the pulmonary function equipment and maintains patients’ files. The CPFT/LPN works independently in the Pulmonary Function laboratory.
The CPFT/LPN will also support patient care in the capacity of LPN as needed.

The Certified Nurse Aide provides direct care to residents to include resident hygiene and activities of daily living under the direction and supervision of a Registered Nurse. The Certified Nurse Aide performs those direct care activities which are allowed to be delegated to unlicensed personnel under the NYS Practice Act, regulations, hospital policies, New York State Long Term Care regulations and standards met by Nursing Services. The Certified Nurse Aide also performs support functions for the unit/department as assigned by the appropriate Registered Nurse and/or manager.

The nurse directs and guides the delivery of nursing care, patient education, and all care related activities, while maintaining standards of professional nursing collaborating with other disciplines as appropriate.

The patient care team on Medical/Surgical and Progressive Care inpatient units consists of a Registered Nurse (RN) and a Clinical Assistant (CA). Matched for a shift, this team provides care for four to six patients depending on unit and shift. CAs are an integral part of hands-on patient care. Working under the direction and supervision of a RN, they are responsible for taking and recording vital signs, performing designated tests and collecting patient samples, and supporting patients in all activities of daily living. CAs meet patients and their families as they enter the unit, welcoming and acclimating them to the unit. CAs interact with unit staff, physicians, and other hospital department personel. It is critical that CAs provide timely and accurate communication, accurate documentation and maintain a harmonious, team oriented, customer friendly environment. CAs must be able to function competently in a computerized medical setting.
The three principal duties of this position are:
- Patient Care: works under the direction of RN Partner to provide responsive, service oriented patient care
- Testing: conducts various patient tests and specimen collection as directed by RN Partner
- Clerical: functions as the “front door” to the unit providing customer service oriented communication and clerical support.

The Director, Partner Engagement will be the regional lead to support the Delivery system Reform Incentive Payment (DSRIP) Population Health Network, for the Plattsburgh region. In collaboration with AHI leadership, coordinates the deployment of resources to determine strategies for achieving quality outcomes across the region.
Accountable Care Organization (ACO)
The Director, Partner Engagement will support new and existing ACO programs and integration with all operations functions. Working with the leadership, develop strategies for program implementation with regional ACO partners, that support population health efforts and value based payment initiatives.

This position demonstrates competency in assessing, diagnosing and treating a full range of patients in accordance with delineation of privileges; Demonstrates ability to organize and prioritize patient care activities; Selects and interprets laboratory and diagnostic testing; Communicates medical assessment and plan to patient as necessary and completes medical record documentation in a timely manner. The primary responsibility is the timely assessment/tx/dispos of patients present to the Emergency Department Fast Track.

Under the direction of the Patient Registration Manager, the Healthcare Access Associate must protect the ability to collect for services performed by accurately interviewing and collecting patient demographic, clinical, and financial information and recording it into the hospital's information system. The Healthcare Access Associate is also responsible for distributing required patient information in a confidential manner, taking admission reservations from physicians' offices and the Emergency Department, and scheduling the appropriate tests, admissions, and communicating with ancillary and clinical departments when necessary. The Healthcare Access Associate is responsible for all telecommunication representative duties during the hours of 1030p – 7a for relief of 30 minute, 15 minute and restroom breaks.

The Clinical Operations Supervisor is responsible for working collaboratively with the Director and other discipline directors, as applicable, to plan for, supervise and evaluate all program operations and patient care delivery in the clinical area. The individual displays or is developing competence in the 5 core health competencies of interdisciplinary teamwork, quality improvement, evidence-based practice, patient-focused care, and informatics. Responsibilities include organizational planning, compliance with regulatory standards, fiscal management, ensure units are staffed appropriately a all times, recruit staff as needed, review and post a 4 week schedule monthly, setting and maintaining standards of nursing care and practice, coordinating services, providing for the professional growth of staff, and clinical coaching on the unit and the bedside with all staff. He/She shares joint responsibility with Director for all outcomes of unit to include but not limited to patient satisfaction, employee engagement and fiscal responsibility. Clinical Operations Supervisors are expected to work holidays and weekends as needed.
